Nurse
Practitioner (NP)
at the Bridge Clinic
Purpose of Position
The Edmonton North Primary Care Network (PCN) is working to bridge
unattached patients from various programs (including the emergency
department at the NECHC, Home Care and self referral) to family
practice. This role will be key to building an effective bridge
from these various settings to primary care.
This role will be critical in delivering appropriate, effective
clinical care (an anticipated 75% of the role), as well as coordination
with key stakeholders and relationship building with family doctors
(10%), program management and evaluation (10%), and education
(5%).Key Responsibilities
The key responsibilities of the NP at the Bridge Clinic include,
but are not limited to:
Be a key stakeholder in the process working with the MOA
partnered to assist with this project to ensure objectives are
met, processes are followed and defined outcomes are achieved.
Ensure that patients are followed up and comply with standard
investigations and treatments for presenting medical concerns.
Target population have difficulty accessing appropriate care in
a timely manner with the right provider.
Coordinate service and provide relevant follow up and resources
for unattached patients and place them with a PCN member physician,
ensuring a smooth transition of care.
Provision of both advanced practice nursing as well as
extended health services. The NP will be a front line
representative in delivering primary health services in the community
and the health centre. Facilitate and provide high quality, comprehensive
and integrated health services to appropriate individuals, families,
groups and the community. This includes individual and team efforts
addressing health promotion, health education, treatment of illness
and injury, as well as prevention of illness and injury; supportive
counselling, preventive and screening measures, referrals and
evaluations as appropriate. The NP is expected to practice at
the advanced end of the nursing scope of practice.
Support and actively participate in program evaluation,
research and clinical teaching activities.
Qualifications & Expertise
Masters in Nursing, completion of an advanced nursing practice
nursing program that is recognized by CARNA, CPR certified, current
registration with CARNA, a minimum of 3 -5 years in community
focused or ambulatory advanced practice nursing.
Sound basis in nursing and community based health service
delivery that includes an understanding of the growth and life
span milestones and issues; determinants of health, Primary Health
Care, health beliefs and practices of various cultures. Knowledge
of the health care system with emphasis on the programs available
through the PCN North. Knowledge of community agencies and strategies
to access and mobilize community resources. Knowledge of the Faculty
of Nursing curriculum.
Strong understanding of primary care and of Primary Care
Networks is an asset
Passionate and enthusiastic about the concept of supporting
patients between acute and primary care; prepared to persevere
with those who are reluctant to change the status quo
Innovative in the delivery of primary care

Early morning, or late evening, meetings may occasionally be required,
particularly if physicians are involved.
This can be a contract or employee role, whichever is preferred.

The Lodge Manager is responsible for the management, administration
and day-to-day operation of Lodge Services including the delivery
of quality resident services, leadership and support to Lodge
staff and the coordination and support of volunteers and contract
services.
PRIMARY ACCOUNTABILITIES
Accountable for the effective management, administration and
day-to-day operation of Lodge services.
Participates in and supports the planning, evaluation
and reporting process for Lodges
Participates in roundtable discussions with other lodge
managers on a regular basis
Participates in overall mission delivery for CCS BCY
Develops and administers the Lodge budget of approximately
$1 million
Identifies opportunities and develops initiatives for
collaboration with internal and external contacts to promote
and market Lodge services
Recruits, promotes and supports donor development through
the provision of Lodge tours, presentations, acknowledgement
of donors and procurement of resident testimonials
Responsible for ensuring that facility (building) operations

Collaborates with the Lodge Volunteer Coordinator(s)
in the recruitment, development and retention of volunteers
and ensures that Volunteer Policies are implemented
Cultivates positive working relationships with Lodge,
Freemasons, Regional and Divisional volunteers, CCS and contract
staff through open dialogue and cooperation
Develops and implements risk management policies including
disaster planning, Pandemic plans etc., and ensures that all
Lodge workforce are regularly practiced in safety procedures
Monitors, evaluates and offers feedback regarding services
provided by contract staff.
